To provide residential placement for transitional age youth 17+ years old (male and female) who have been diagnosed as having in intellectual disability and who are DSO eligible. The home will provide a family-like environment with private bedrooms and will support five to eight youth. This model will provide more opportunity for both individualized and structured support for the youth with moderate levels of support requirements. The staff skills and service capacity would also exist to consider youth who require more significant levels of support due to more complex and multiple needs. This would be accomplished using an individualized plan approach to identify the additional resource requirements ideally prior to the start of placement.
