Cobourg Transitions – Provides a residential treatment option for individuals who require supports for 24 hour assisted living. The Program has three treatment beds and is located in a quiet neighbourhood. Clients are assessed and supported by personal support workers. The facility is staffed 24 hours a day. Meals are prepared by the personal support workers,or by the adults themselves - program is compliant with QAM - each person has their own ISP.
